- Gather Record of Birth: The most common option is a birth certificate. If your child was born in Tennessee, you can request a birth certificate by visiting the Union County Health Department. If your child was born in another state, you'll need to contact that state's vital records department. Other acceptable documents include a valid passport issued by any nation (with translation if needed), immigration documentation, a decree of adoption, or other official records issued by a court.
- Get Immunization Records: Schedule an appointment with your child's doctor or the Union County Health Department to get required vaccines and ask for a copy of your child's immunization records.
- Gather Proof of Residence: Common examples include a utility bill or a lease agreement. (In cases where these are not in the name of a parent/guardian, a notarized letter stating the parent/guardian and child(ren) are living at that residence.)

At Union County Schools, all registration and enrollment takes place at the student’s school and must be completed by a parent or legal guardian.
All forms must be filled out completely and printed along with the following documents:
- Proof of Birth: Government-issued Birth Certificate, passport, immigration documentation.
- Proof of Custody (if applicable): Decree of adoption or other court-issued records
- Immunization Records: The student’s Tennessee Department of Health Immunization Certificate (shot record signed by the child’s physician or county health department).
- Proof of Residence: One of the following—gas, water, or electric bill. If the utility bill and/or lease agreement is in the name of someone other than the parent/guardian, that individual must provide the utility bill and/or lease agreement, along with a notarized letter confirming that the parent/guardian and children reside at the address.
- Copy of Guardian DL: A copy of at least one of the parent/guardian's driver's licenses.
- Name and Address of Previous School: The new school will send a formal request for your child's records.
Bring the completed, printed enrollment packet and all required documents to your zoned school. Some schools have specific times for dropping off enrollment forms, so please check the school’s website or call the main office for details. No enrollment materials will not be accepted by email.
- Proof of Birth: Government-issued Birth Certificate, passport, immigration documentation.
